How to register your clinic on ABDM’s Health Facility Registry (HFR): the documents, the steps, how long it takes, and what your facility ID unlocks. It’s free.

If you run a clinic or hospital in India, an HFR facility ID is fast becoming the front door to everything digital-health — linking patient records, joining ABDM-backed schemes, and meeting state directives that increasingly ask for it. The good news: registration is free, done online, and most facilities finish in under an hour if their papers are ready. Here’s exactly how.

What HFR is (and why you need it)

The Health Facility Registry (HFR) is the national list of verified health facilities under the Ayushman Bharat Digital Mission (ABDM), run by the National Health Authority (NHA). Whether you’re a single-doctor clinic, a diagnostic centre, a pharmacy or a multi-specialty hospital, you can hold an HFR entry with a unique facility ID.

That ID is a prerequisite for almost everything else in ABDM: it’s what lets your software link a patient’s visit to their ABHA health account, appear in national facility searches, and take part in programmes built on ABDM rails. One important thing to be clear about — HFR does not replace your local trade licence, state health-authority registration, or NABH. It sits alongside them.

Before you start: what to keep ready

Registration goes quickly when the details are on hand before you log in. Gather:

  • A mobile number linked to Aadhaar for the person registering — you’ll verify by OTP.
  • Your facility’s basic details: name, full address with PIN, ownership type (government/private/trust), and facility type (clinic, hospital, lab, pharmacy).
  • Operational details: system of medicine (allopathy, ayurveda, etc.), number of beds if any, and the services you offer.
  • Supporting documents your facility type asks for (for example, a registration or establishment certificate). Requirements vary by state and facility type, so check what the portal lists for you.

The steps, start to finish

1. Open the ABDM facility portal. Go to the Health Facility Registry on the NHA’s provider portal (facility.abdm.gov.in). If a link ever 404s — government portals move — search “ABDM HFR registration” from the official abdm.gov.in and follow the link there.

2. Create or sign in with your ABDM provider account. Facility registration runs through a provider (HPID) account. Create one for free with your Aadhaar-linked mobile, or sign in if you already have one, then choose the facility manager role.

3. Enter your facility details. Fill in the name, address, ownership and facility type, and the operational fields above. Take care with the address and PIN — this is what patients and other facilities will see in national searches.

4. Add services and upload documents. Select the services you provide and upload the supporting documents the portal requests for your facility type.

5. Submit for verification. Review and submit. The application goes to the relevant authority for verification.

6. Receive your facility ID. Once verified, your facility gets its unique HFR ID. Keep it safe — this is the ID your hospital software will use to connect to ABDM.

How long it takes, and what it costs

The NHA charges no fee to register a facility on HFR. Filling the form takes well under an hour with documents ready; verification is a separate step that can take anywhere from a day to a couple of weeks depending on your state and facility type. There’s nothing to pay to anyone at any stage — if a “consultant” asks for a registration fee payable to the government, that’s a red flag.

What your facility ID unlocks

With an HFR ID in hand, you can move on to the parts patients actually feel: creating or linking a patient’s ABHA at the front desk, and — with the patient’s consent — sharing a clean digital record of the visit. That record travels in the national FHIR format, so it’s readable by any other ABDM-connected facility the patient visits next. Common mistakes to avoid

  • Registering with a personal mobile that isn’t Aadhaar-linked — OTP verification fails. Sort the Aadhaar link first.
  • A mismatched or sloppy address — it becomes your public listing, and it should match your other registrations.
